Practical evidence about day-to-day function
Form 1 and Attendant Care
For Ontario automobile accident claims, the Assessment of Attendant Care Needs — Form 1 — identifies and quantifies accident-related attendant-care needs. Form 1 has to be completed by an occupational therapist or registered nurse.
Assessment Scope
The assessment may look at the person's caregiving responsibilities before the accident, the dependant's needs, the physical, cognitive, or psychological demands of the caregiving role, the accident-related restrictions, and the assistance now required.
Caregiver Benefit Assessments
Caregiver benefit is separate from attendant care. It concerns an insured person's inability to keep providing care to a dependant because of an accident-related impairment. An assessment may look at the person's caregiving responsibilities before the accident.
Assessment Process
Since July 1, 2026, medical, rehabilitation, and attendant-care benefits remain part of mandatory Ontario auto insurance coverage. What's actually payable in a given claim still depends on the SABS, the policy, the accident date, the evidence, and which expenses qualify.
